Plastic surgeons in the UK commonly use silicone implants – they are the most popular because they are safe, made to last and look and feel natural.

The alternative is saline implants, which contain sterile saltwater, but they are firmer than silicone versions, more perceptible, and deflate fast should the shell break.

Kevin Keller, a US surgeon, invented the device in 2009 to prevent the implant from touching the skin and keep it sterile – which is why it’s often called the ‘no touch’ or ‘minimal touch’ technique. The funnel is FDA (Federal Drug Administration) approved in the US and is a brand of Allergan, which is now an AbbVie company.

Your surgeon takes your implant straight from its packaging, loads it into a Keller Funnel®️2 gently propels it through the incision to the chest pocket.   • Breast implant rupture – rare nowadays, but it is a complication of breast augmentation surgery. Rupture means the implant shell breaks. Signs that your breast implant may have ruptured include breast shape and size changes, increasing pain and firmness, and swelling.
  • Capsular contracture – fibrous tissue forms naturally around a breast implant, creating a protective tissue capsule that keeps the implant in place. It should feel soft or slightly firm and go unnoticed. However, sometimes the formed capsule is hard and dense, tightening and squeezing the implant. Signs that you may have capsular contracture include chronic pain and distortion in the breast shape, growing firmness or tightness in the breast and the breast rising higher on the chest.

Everyone is different, so the amount extracted varies per patient. However, six litres are the most fat we can remove in one procedure.

Before PAL’s introduction in 1998, surgeons had to physically move cannulas to and fro to suction out body fat. It was tiring for the surgeon and often left some fat in place.

Using PAL’s fast-vibrating cannulas instead of a traditional energy-based cannula reduces the effort it takes to suck out the fat, and it doesn’t use heat. As a result, PAL makes the process faster, more efficient and gentler for the patient, and more fat can be removed.

PAL also allows surgeons to sculpt body areas like the chest and stomach, which is particularly beneficial for men who want better contours.

These latest devices use energy-based lipolysis or fat-melting instead of suction to remove fat deposits and reduce the possibility of bruising caused by other methods.

However, our surgeons were unimpressed by the trials, which found that such devices can lead to skin burns, hardening (fat necrosis), unevenness and patient disappointment with the outcome compared to PAL.

There is no substantial proof that bruising is reduced, either. However, our highly skilled surgeons always minimise bruises, lessen pain and encourage healing.

When comparing energy-based devices to PAL, our surgeons have found no difference in bruising, swelling, post-procedure pain or recovery times.

Recovery time can vary between patients but is mostly dependant on the amount of fat extracted. Liposuction is minimally invasive, but the more fat removed, the longer the recovery time is likely to be.

No, we have found that the operation time with the new energy-based liposuction techniques can be longer.

A shorter general anaesthetic, achieved with PAL, is best to improve patient recovery and reduce the risks and complications that may happen with any surgical procedure.

  • Grade 2a – moderate breast enlargement and no excess skin – breast growth expands across the width of the chest and can be seen through skin-tight clothing such as T-shirts, causing distress. Breast surgery reduction is advised
  • Grade 2b – moderate breast enlargement and excess skin – the breast tissue expands significantly beyond the chest width and has some extra skin that’s noticeable through clothing
  • Grade 3 – significant breast enlargement and excess skin – the breasts are visible and look like female breasts. Surgery will reduce them to average size and shape.

There are three botulinum toxin type-A medicines available for cosmetic use in the UK:

They work by temporarily relaxing specific muscles, effectively preventing them from creasing or wrinkling the skin.

We have experience with all three formulations at The Levant Clinic but mainly use Azzalure. We don’t suggest that one brand is superior to another; inevitably, it’s simply the practitioner who chooses their preferred product. The UK’s MHRA and the FDA in the USA regulate Azzalure.

During your consultation, we’ll tell you which botulinum toxin product we will use in your treatment.

On average, injectable anti-ageing dermal treatments last three-six months, depending on the product used and the area treated. After six months, your body naturally breaks down the botulinum toxin type A, so you’ll need to get touch-ups to maintain your desired look.

You’ll see a difference in the treated areas within three to five days and will continue to see improvements in the following two weeks.
